Health Insurance
In Japan, there are three types of public health insurance programs for medical treatmentWorkers’ Health Insurance (health insurance programs for company employees), the National Health Insurance program (Kokumin Kenko Hoken) for local residents who are not enrolled in an employee health insurance program, and the Long Life Medical Care System (also known as the Latter-Term Elderly Medical System) for citizens 75 years of age and older. If you plan to live in Japan for one year or longer, you must enroll in some type of public health insurance.
How to Enroll
Please check with your place of employment concerning health insurance for employees. The enrollment procedures for National Health Insurance are as follows:
Bring your foreign resident registration card to the City Office and complete the application procedures. However, those whose period of stay has expired or have a short-term visa, and those who do not plan to live in Japan for one year or longer, are not eligible for this insurance program.
Notification of Changes After Enrollment
- If your address in Shinagawa changes.
- If the head of household changes or your name changes.
- If a child is born.
After completing foreign resident registration for the child, please bring your National Health Insurance card and complete a notification form.
- If you enroll in the health insurance program at your place of employment.
Please bring the health insurance card from your place of employment and the National Health Insurance card and complete a notification form.
- If you move out of Shinagawa.
Please return your Shinagawa National Health Insurance card to the City Office as soon as possible. You must complete enrollment procedures for NHI again at the municipal office of your new residence.

Long Life Medical Care System (Latter-Term Elderly Medical System)
Those who are 75 years of age or older (65 years of age or older for those with a designated level of disability) must enroll in the Long Life Medical Care System, which was established in April 2008. However, those whose period of stay has expired or have a short-term visa, and those who do not plan to live in Japan for one year or longer, are not eligible for this insurance program.
